Cancel internship in 2017
Radically changed the scheme of study in medical universities in Russia.Due to cancellation of internshipmany fear that dropouts will come to the clinics. The introduction of new medical standards has divided doctors into two grades.
From 2016, future local pediatricians and general practitioners begin to study according to an abbreviated program, which implies six-year education. Potential surgeons, cardiologists and other medical specialists will have to spend from 8 to 11 years to study.
As it was
Previously, all the doctors studied for seven or eight years. Six years went to study at the medical institute, and then either an internship for a year, or a two-year residency. Moreover, in some specialties it was necessary to go through both these steps. However, for many medical specialists it was enough to have one internship or residency to choose from.
Intern and resident are considered in the medical institution as something like a student, as they do not count on the staff, and they do not receive salaries. They are engaged in essentially the same as the doctor, with only one difference - all actions are interns and interns under the watchful eye of senior colleagues.
It is not surprising that many, trying to quickly get rid of this half-status, choose a one-year internship further after the university. However, in 2017, medical graduates will no longer be tormented by a similar choice.
From 2017, students of medical universities will be taught according to new educational standards. Immediately after graduation, graduates will be eligible immediately, without any additional training, to work at the policlinic sites (therapists or pediatricians). For these purposes, much more practical training was introduced for students, both with patients and simulators.
Rector of the First Moscow State Medical University named after I. M. Sechenov, Professor Peter Glybochko, shared his thoughts on how the new system of education will be implemented:
“Now, the first year is supposed to be an educational practice for the care of patients of a therapeutic and surgical profile. In this regard, it is planned to introduce the training module “Basics of patient care” in the amount of 36 hours (24 hours will be allocated for practicing skills on models and simulators in our own training center and 12 hours for practice in clinics).Second-year students will learn “the basics of nursing” in the amount of 72 hours, they will also have work and practical training in a training center ”.
Third-year students want to add hours of practical courses in emergency medical care. For fourth-year students and fifth-year students, they are going to develop additional training modules at specialized departments of surgery, therapy, obstetrics and gynecology.
And here also it is impossible to do without simulators and models. In the sixth year, classes in medical simulators will be held in the form of consultations with teachers of the departments of the chosen profile.
“A graduate of a medical higher education institution should be ready for independent work immediately after obtaining a medical degree. Let it be the primary link of public health - the outpatient polyclinic sphere - but our graduates will be prepared for this work already in the course of a standard university program, ”concluded the Rector of the First Medical University.
However, not all teachers in the field of medicine see such bright prospects. Simulators and models are, of course, good, but living patients cannot be replaced by them.
“In my opinion, immediately allowing graduates to work with patients will be a big mistake. The doctor needs certain skills for this, ”said Victor Frolov, dean of the medical faculty of the PFUR, expressed his concerns.
Also, the overwhelming majority of specialists seriously doubt the productive work of yesterday's students in polyclinics. It is unlikely that the skills acquired at the university will be enough, but graduates will have even less knowledge after the introduction of new standards.
“In order to free up time for practical training, we had to narrow the programs in certain areas of medicine, such as neurology or, for example, surgery. These courses will be taught in a more general, theoretical form, ”the Ministry of Health and Social Development explained.
It turns out that if earlier the medical graduate was something like a “universal soldier” who could understand most diseases and pathologies with the help of a medical reference book, now certified local doctors about many diseases will have only a theoretical idea.
“It's too bad that graduates will be able to heal right after high school. Our doctors have insufficient training, ”said Vasily Vlasov, President of the Society of Evidence-Based Medicine Professionals.
The new model of medical education can greatly shake the health sector. We, ordinary philistines, can only hope that the officials know better and they act according to the mind.
